Root Cause/Incident Investigation Workshop
This intensive five-day workshop in root cause analysis is a condensation of the core training for the U.S. NRC's IITs and AITs, the Department of Energy, the Chief of Naval Operations safety function, a number of commercial utilities, and selected Australian government agencies. The course outline includes five root cause analysis techniques, the "model" root cause program, investigative interviewing, human performance, report preparation and presentation and guided practice in performing an incident investigation.

Root Cause/Incident Investigation Refresher
Designed for those who have attended a MORT or MORT A/I course but who have not had extensive practice in using analytical techniques, this course is also helpful for managers and reviewers who have had exposure but no training in MORT and other root cause approaches. The course includes brief discussions of topics covered in the MORT A/I Workshop and guided practice.

Safety Behavior Observation Program
The basis for changing behavior is knowing how to perform good observations. The five-step process taught during the course assures improved safety, reduced errors and a better understanding of the job being performed. Videotape exercises are used to provide practice.

Management Safety Assessments
 This three-day workshop includes four analytical techniques used to evaluate the management system of an organization. This process has been taught to NRC, Navy and others. The Management Evaluation and Risk Identification Tree (MERIT) is included.

Safety Analysis Software Training
This two-day short course teaches the fundamentals of using the Conger & Elsea, Inc. MAPS software. Familiarity with the techniques is a plus. All participants receive a licensed copy of the software.

Introduction to Root Cause Analysis
This two-day workshop is designed for those who will investigate incidents of low to moderate consequence. Three analytics are presented - Change Analysis, Hazard-Barrier-Target, Events & Causal Factors. Interviewing and reporting basics are treated. Case studies are used.

Investigative Interviewing
This three-day workshop teaches how to do information gathering interviews. Example discussions, role plays and guided practice using individual videotaping sessions and feedback help develop individual skills.
