FLOWCHARTING, FAULT TREE ANALYSIS,
and EVENTS & CAUSAL FACTORS all use the same basic operating steps. Want to be able to draw a process flowchart,
computer flowchart, or an organization chart? CLICK, draw a box. The same simple
process is used to draw connecting lines. Work process flowcharts are often used
during process safety audits and appraisals. All are easy to learn. In less than
one hour, you can be drawing basic diagrams. If you know how to operate one, you
can operate them all. No more reasons for out-of-date charts. EVENTS & CAUSAL
FACTORS ANALYSIS gives you a way to draw EVENTS & CAUSAL FACTORS charts during
investigations. Select one symbol at a time or create a macro which includes a
series of symbols. You can draw events, conditions, and presumptive events or
conditions. Bold lines are used for primary lines, thin lines for secondary lines,
and dashed lines for conditions. You can quickly and easily create or modify even
complex diagrams of any size. These diagrams can be saved and later recalled.
You can even combine two diagrams into a new single diagram. In the end, you will
have a complete and detailed picture of who did what, when, where, how, and, most
importantly, WHY?
CHANGE ANALYSIS Like the cartoon - "What's
wrong with this picture?", CHANGE ANALYSIS prompts the user to look for the
causes of problems in changes that have occurred. By laying out information about
the accident circumstances and safe circumstances in parallel, differences are
identified and listed. Finally, those differences are analyzed for their effects
on the mishap. You can also use CHANGE ANALYSIS to evaluate the effects of a potential
change (under consideration). A thoughtful evaluation of modifications and changes
means fewer problems later. This is a very easy technique to learn and is also
very efficient. When completed, a spreadsheet style printout is available.
HAZARD-BARRIER-TARGET ANALYSIS
This
is a quick, system-oriented analytical technique set in the form of a spreadsheet.
The user is prompted to identify what was damaged or who was hurt in the mishap.
Then, you identify what hazard was involved. Now the work begins. The barriers
intended to prevent the mishap are identified, listed and analyzed. Each barrier
is coded as to where it fits on the safety precedence sequence. Then each barrier
is classified as to: did not fail, failed, did not use, or did not provide. Printouts
suitable for report inclusion can be produced. This technique by itself is often
sufficient for "minor mishaps".
FAULT TREE ANALYSIS
The analysis of
hardware failures is often assisted by the use of qualitative FAULT TREE ANALYSIS.
A fault tree provides a detailed picture of all the different failure scenarios.
The user identifies the critical path to failure. FAULT TREE ANALYSIS provides
the user with the symbols, connecting lines, and logic gates needed. You can quickly
and easily produce or modify the tree. Because they can be saved, you can create
a "library" of trees to be used later. A printout of the tree can be
generated for demonstration or report use.
OPERATIONAL READINESS/APPRAISAL
TREE Starting a new or modified facility requires coordinating many elements
and attending to many details. The OPERATIONAL READINESS/APPRAISAL TREE can help
you make sure that ten major areas are not overlooked. Comprehensive "checklists"
are provided for evaluating safety in each of the following areas: Industrial

MANAGEMENT EVALUATION AND RISK IDENTIFICATION
TREE (MERIT) is the application of the MORT technology to evaluate a system
before an accident/mishap happens. MERIT is used to find holes in the management
system. MERIT was recently used on a project to find the strengths and weaknesses
in a licensing process by a federal agency. The software walks the user systematically
through a series of comprehensive questions. The depth and breadth of an evaluation
can be established by predetermining which levels of questions are answered. If
